What is the max reimbursement amount?

Please look at the maximum amount you can get reimbursed for in your offer letter.

Can I request reimbursement more than once a month?

You may only request reimbursement once per month, either on the 5th or the 20th. If you forget to request reimbursement, you may not request reimbursement twice during the same month.

What type of receipts do I need to submit?

This is a reimbursement request, which means we will need PAID receipts. If the paid receipts do not have clear information about what you request reimbursement for, you must attach the original bill.

Can the receipts be under my spouse's or parent's name?

All reimbursements MUST be under your name. No spouse or parent names are allowed. NO exceptions.

What falls under the health reimbursement?

Some items include, any type of medical procedure including dental work, gym membership or fitness class, vitamins or medicines. If you have any questions about what falls under the health reimbursement, please contact Maria.

Where do I submit supporting receipts if I can't send it through this form?

Any supporting receipts or documents should only needs to be sent to [email protected]

What is the deadline for submission?

Please send the reimbursement sheet with proof up to one day before payroll. It will only be reimbursed if you remember to send it before payroll is sent out.

Will someone send a reminder so I don't forget to submit my reimbursement?

No one will remind you to submit reimbursement. It is up to you to remember.

How do I fill out the sheet when it comes to dollar amounts?

On the reimbursement sheet, only write the maximum amount on the sheet, so if your max amount for internet is $20 but your internet bill is $30, on your sheet, you will write $20 as that is your max.

I missed sending my reimbursement for the past few months, may I submit now to reimburse the back dates?

No, you reimbursements are use it or lose it. You may not submit reimbursements for the past few months if you missed it.
